Problem
The prior PR could not be viewed/ UAT-ed, due to a lack of sample data to populate the endpoint that had been created.
Solution
This PR introduces sample data that allows for the OrganizationAffiliation endpoint to be spun up and show underlying records.
Result
Adds additional endpoint_instance records, as well as location_to_endpoint_instance records to the sample data repeatable migration file.
Test Plan
make upand ensure that the migration executes successfully (i.e. that command should not error out)http://localhost:8000/fhir/OrganizationAffiliationand confirm that data appear
